How can I access Summit Health Solution Services?
In Home
In Home

If they have an HCP, we can coordinate with their Care Manager to provide services at home. Once they send a referral, we'll handle the rest and book the sessions.

How can I pay?

You can access our services and programs in different ways, including the funding options below.

Government Funding
Government Funding

If your loved one has Home Care Package (HCP) / Support at Home funding, their Care Manager can arrange services for them with us by submitting a referral on our site.

Private Pay
Private Pay

You can pay privately if your loved one does not have Home Care funding yet. Please send us a direct email or call on 1300 315 315
