Meet Grace:
After a hospital stay, Grace found it difficult to shower and dress on her own. Her son Michael noticed she was skipping showers, wearing the same clothes, and becoming more withdrawn. Worried about her safety and self-esteem, Michael contacted Summit Health Solutions for support.

Together, we put a Personal Care plan in place: gentle help with showering, grooming, and getting in and out of bed—always guided by Grace’s preferences. Over time, she felt more like herself again: fresh, comfortable, and confident to have visitors.

When to Arrange Personal Care Support

Personal Care is ideal for older adults who want to stay at home, but need a bit of extra help to manage safely and comfortably.

Is your parent missing medication or feeling embarrassed about continence issues?

Our team can:

  1. Provide medication reminding and prompting from a Webster pack
  2. Offer basic assistance with medication only where workers are suitably qualified and it is appropriate within your care plan
  3. Assist with toileting and incontinence care, including changing incontinence products and maintaining hygiene
Getting Personal Care Through Home Care Packages

If your loved one receives funding through the Australian Government’s Support at Home program/HCP, Personal Care is often one of the most important supports in their plan.

Under Support at Home, your loved one may be able to use their individual budget for:

Assistance with showering, bathing and toileting
Help with grooming, dressing and undressing
Support with mobility and transfers in the home
Supervision and prompting for personal hygiene and daily routines
Medication prompting and reminders from a Webster pack (within worker qualifications)
The new funding model means the government covers a significant portion of the cost of approved services through your Support at Home budget. Depending on your income and circumstances, you may also contribute a co-payment for some non-clinical day-to-day supports, such as personal care.
