What is Summit in home nursing and what can it offer your client?

A referral to Summit in-home nursing will assess areas of your client’s overall health that may be impacting their independence and quality of life. Nursing can provide support and assistance to clients, as well as help them to implement preventative health strategies in order to improve or maintain a client’s current health status. This assistance can be provided by either in–home nursing visits or telehealth appointments.

Following our initial Clinical Assessment which is conducted in the clients home by our AHPRA registered Registered Nurse, a care plan is created for ongoing services.
